CHAMP Performance Indicator Definitions
This document describes the performance outcome that are found in the CHAMP reports. The performance outcome reports can be found in CHAMP on the reports screen under quality management. The definitions below describe the data found in the numerator and denominator and any data that may be excluded. Superscripts can be found at the end of the document. Definitions are listed in the order they appear on the report.

In CHAMP, this is represented by a “YES” answer with a valid test type to Question 8 - TB Screening on the medical tab.

Influenza Immunization (2013) ID: 1067 Category: 2. All Ages

Patients who received an influenza immunization OR who reported previous receipt of an influenza immunization during the current season.
Previous receipt can include: previous receipt of the current season’s influenza Immunization from another provider OR from same provider prior to the visit to which the measure is applied (typically, prior vaccination would include influenza vaccine given since August 1st). All patients aged 6 months and had a medical visit with a provider with prescribing privileges1 at least once in the measurement year
In CHAMP, this is represented by answering “YES” to question 24 if the vaccination has been given according to the requirements above.
